Those who have lost/misplaced their MV278 (5 hr Prelicensing certificates) or MV285 (Driver Education Certificates) and need replacements, must pay $5 for a replacement.  Once fee is paid, email:  kturcio@acsdny.org and advise that you have done so.  Arrangements will be made to obtain the certificates.
